The Canadian Medication Incident Reporting and Prevention System (CMIRPS) is a collaborative pan-Canadian program of Health Canada, ISMP Canada, Canadian Institute for Health Information, Patients for Patient Safety Canada and Healthcare Excellence Canada. The goal of CMIRPS is to reduce and prevent harmful medication incidents in Canada.

Reporting, sharing, and learning about medication incidents all aim to reduce these incidents’ reoccurrence, and to help create a safer healthcare system for patients and healthcare providers.
The Ministry of Long-Term Care (MLTC) in Ontario is partnering with ISMP Canada for 3 years to support long-term care homes in strengthening medication safety.
Spearheaded by the Canadian Patient Safety Institute and the Institute for Safe Medication Practices Canada and built from discussions with industry, the Canadian Medication Safety Coalition formed in 2017 with the aim of boldly accelerating improvement to Canada’s medication-related systems.
